Orders due no later than MONDAY 10/2 (must be turned into school on Monday, or this on-line form filled out - do not do both)

Order Pick up is on FRIDAY 10/6 between 2:00pm and 3:30pm at one of these locations:

Lamonte Annex
Lafayette
Smalley

You will indicate the school you will pick up at on your order form

If your order is not picked up, it WILL be donated to a local cemetery. We kindly ask that you do not place an order if you are unable to pick up at the designated times/day
